You can use the link below to reach the Information Platform prepared by the Turkish Ministry of Health concerning COVID-19 vaccination which is an essential tool in the fight against COVID-19 pandemic which has been prevalent throughout the whole world for more than a year now, having claimed about 4.2 million lives and adversely affecting the social and economic life.

As TED University, we attach great importance to the fight against COVID-19 pandemic throughout the world. In this context, we are supporting the scientific vaccination activities aiming to reach a communal immunity and we also expect our dear members to kindly support these activities.

At this stage, we kindly ask you to fill in the updated Health Information Form given below, attach your COVID-19 Vaccination Card to the form and submit them in order to determine the current status of vaccination in our University, which has been about 75% throughout Ankara. 

We kindly ask you to fill in this form honestly in order to help protect the public health and ensure implementation of preventive medicine, medical diagnosis, care and treatment activities which will reduce the risk of exposure and spread of the virus. Thank you for your support.

 

Please check the validity date of your HES Code. (It is expected to be valid for at least 6 months.) If your HES code is not valid, create it for yourself. (E-Government Gateway / HEPP Application)

HES (Life Fits into Home) Code allows you to share your health condition in terms of COVID-19 safely with other people and institutions for transportation and visiting purposes within the framework of controlled social life.

Clarification Text on PDP

Any personal and private data processed due to COVID-19 outbreak is to be stored limited to the purposes mentioned above, only during the outbreak; these data will later be obliterated either by erasing, destroying or anonymizing. Medical information can only be accessed by the health unit personnel.

In cases when persons have been suspected to exhibit COVID-19 symptoms, only the contact information of the people they have been in touch will be shared with public health authorities. Your rights as mentioned in the Article 11 of Law No.
